When you order an instrument on this website, you will automatically be sent a scoring manual. Please follow the scoring instructions in the manual.

Permission to use any questionnaire from this website is obtained by completing the form on the order page, providing all necessary information including intended use of the questionnaire.

Validated versions of the questionnaires are available in a wide variety of languages. Click here to order the questionnaires by language.

This happens if you do not make a selection under both the "Language" and "Questionnaire" columns. Please be sure to check at least one choice in each column.

Since our questionnaires are sent as email attachments, your email system may block them, believing them to be spam or viruses. Please check your junk mail folders and filters. Also, if you order more than two or three questionnaires at one time, the email generated may be too large for reliable transmission. If you do not receive the questionnaires within 24 hours, please contact us.

Questionnaires or questionnaire items should not be reprinted in a publication. Please cite the appropriate reference (www.pfizerpatientreportedoutcomes.com) in the appendix.

New translations of our questionnaires are considered to be Pfizer’s property and final files should be forwarded on to us in Word and PDF format along with translation certificates.
