How do I score the questionnaires?

When you order an instrument on this website, you will automatically be sent a scoring manual. Please follow the scoring instructions in the manual.

How do I obtain permission to use the questionnaires?

Permission to use any questionnaire from this website is obtained by completing the form on the order page, providing all necessary information including intended use of the questionnaire.

Are the questionnaires available in languages other than English?

Validated versions of the questionnaires are available in a wide variety of languages. Please visit the Disease Areas tab to order the questionnaires by language.

After registering, I received an email but nothing was attached.

This happens if you do not make a selection under both the "Language" and "Questionnaire" columns. Please be sure to check at least one choice in each column.

Reprinting questionnaires or questionnaire items in a publication?

Questionnaires or questionnaire items should not be reprinted in a publication. Please cite the appropriate reference (www.pfizerpcoa.com) in the appendix.

What if a language that I want to use is not listed on www.pfizerpcoa.com as already available?

Please Contact Us if there is a language you are looking for that is not listed on the web site. It may already be in process and soon available for use. If a new translation is required, we will grant permission based on the linguistic validation process below.

How do I conduct translations of measures ordered from www.pfizerpcoa.com?

Translations of our questionnaires should meet standards that are documented in the reference below. New translations of our questionnaires are considered to be Pfizer’s property and final files should be forwarded on to us in Word and PDF format along with translation certificates. Please contact us if you have further questions.
